How it works
From enquiry to start of care.
A clear process that removes uncertainty – for families and for professional partners. Not every request reaches step 6. We tell you openly what is possible.
6 steps
-
Get in touch
A phone call or email. You briefly describe who needs support and what kind of help you imagine. You don't need to prepare anything. An enquiry does not create an agreement.
-
Clarify needs together
What support is needed, how often, at what times? Is there a medical prescription? What documents do the health insurance and care insurance need? We tell you what to do next.
-
Check documents and conditions
We check whether the required documents are available and what conditions the payer sets. If something is missing, we tell you who can provide it.
-
Check care feasibility and logistics
We check whether the care fits our offering, whether the location is within our service area and whether we have capacity. This depends on the need, the location and the timing.
-
Agree on planning
If care is possible, we agree with you: which services, to what extent, at what times. With professional partners we plan discharge or transition together.
-
Start of care
Care begins. If something changes, we adjust the plan together. We stay in conversation.
For hospitals, practices and social services
The process is the same for professional partners. The focus is on the professional consultation: ventilation situation, medical devices, required hours and special features. A separate page for professional partners lists the details.
For hospitals & practicesNot every request reaches step 6
Whether care is possible depends on the need, the location, the conditions set by the payer and our capacity. A start of care is not guaranteed simply because a request has been sent. We tell you clearly whether and to what extent care is possible.
A phone call or email is enough. You don't need to prepare anything.
