For hospitals, practices & social services

Request care.

This page contains the key information for assessing a possible collaboration: range of services, service area, process and contact.

Range of services

What we take on.

Whether we can take on a specific case is clarified individually.

  • Medical treatment care Measures prescribed by a doctor, such as wound care, changing dressings, administering medication and injections. A prescription for home nursing care.
  • Basic care Support with personal hygiene, eating and drinking, and mobility – as independently as possible. The scope depends on the care level (Pflegegrad) and personal needs.
  • Household help Shopping, laundry, tidying – so that everyday life at home keeps working.
  • Counselling visits under section 37(3) SGB XI The regular counselling visit for people who receive care allowance (Pflegegeld). Receipt of care allowance.
  • Out-of-hospital intensive care Intensive nursing care in your own home instead of in hospital. A doctor’s prescription and coordination with the health insurance fund.
  • Out-of-hospital ventilation Care for people on ventilation outside hospital, including tracheostomy care. A doctor’s prescription. Care is planned together in advance.

You will find detailed descriptions under Services.

Process

How an enquiry works.

  1. Enquiry

By phone or email, with the care needs in brief: type of care, approximate scope, place of residence, planned start.

  2. Professional consultation

We clarify the nursing requirements with you – for example the ventilation situation, aids, required hours and special features.

  3. Checking whether care is possible

We check the service area, needs and capacity and give you clear feedback.

  4. Planning the handover

If we can take on the care, we coordinate discharge or the start of care, documents and contact persons with you and the family.

An enquiry does not automatically mean that we take on care. Whether we can provide care depends on the needs, the place of residence and our capacity.
