Out-of-hospital intensive care

Out-of-hospital intensive care is intensive nursing care in a person’s own home instead of in hospital – as one-to-one care, with a nurse present continuously for one person. It also covers the care of people receiving mechanical ventilation.

Who is it for?

  • People who need permanent intensive nursing care and want to live at home.
  • Ventilated people whose care is to continue outside hospital.
  • Families preparing a transfer from hospital to home.
  • Hospitals and social services planning a transition.

What can Rozali’s take on?

  • Monitoring of ventilation
  • Secretion management
  • Tracheostomy care
  • Safe handling of the equipment in use

The owner is a nursing specialist for out-of-hospital ventilation. Which measures apply in an individual case depends on the doctor’s prescription and the decision of the health insurance fund.

What is needed for this?

  • A doctor’s prescription.
  • Coordination with the health insurance fund.
  • Joint planning of the care before it begins.
  • A home environment in which the care can be provided safely.

How an enquiry works

  • You get in touch.
  • If care is a possibility, we look at the situation together.
  • The care is coordinated with doctors and the insurance fund before it begins.

Who pays for it?

Out-of-hospital intensive care is a health insurance benefit under SGB V and is prescribed by a doctor. The health insurance fund decides on the scope case by case.

Frequently asked questions

What does one-to-one care mean?

It means a nurse is present continuously for one person, rather than caring for several people one after another. How many hours this covers depends on the prescription and the decision of the health insurance fund.

Do I need a prescription?

Yes. The basis is a doctor’s prescription, together with coordination with the health insurance fund. We clarify both before any care begins.

Can hospitals plan a transition with you?

Yes. Hospitals, practices and social services are welcome to approach us. A good transition does not begin on the day of discharge – the earlier we know the need, the better we can prepare the planned care together.

Do you also provide care outside Wuppertal?

Our service area is Wuppertal and the surrounding region within roughly 50 kilometres. For intensive care in particular, admission additionally depends on the individual case and on capacity.
