For hospitals & practices Requests to take over care

When you can call us

Perhaps it is one
of these situations.

  • Someone is coming home from hospital and needs continued care there.
  • Ventilation is to continue at home rather than on a ward.
  • A dressing needs changing regularly, and the practice is too far away.
  • Caring at home is becoming too much for the family alone.
  • The counselling visit required for the care allowance is due.
  • You do not yet know what you need and would like to ask first.

If you are unsure whether we are the right service for you: call and ask.

Services

How we
can support you.

Which service is right for you depends on the doctor’s prescription and the assessment by the insurance fund. We clarify this with you.

Medical treatment care

Measures prescribed by a doctor, such as wound care, changing dressings, administering medication and injections.

Basic care

Support with personal hygiene, eating and drinking, and mobility – as independently as possible.

Household help

Shopping, laundry, tidying – so that everyday life at home keeps working.

After your enquiry

What happens next.

It starts with a question that commits you to nothing.

  1. You get in touch – without obligation

A phone call or an email. Briefly describe who needs care. You do not need to prepare anything. Making an enquiry does not create any agreement.

  2. We clarify the needs together

What support is needed, how often, at what times? Is there a doctor’s prescription? Which documents do the health or long-term care insurance funds need? We tell you what to do next.

  3. We check whether we can take on the care

This depends on the needs, the place of residence and our capacity. We tell you clearly whether and to what extent care is possible.

  4. We get to know each other

If care is possible, we talk in person, look at the situation and answer your questions.

  5. Care is planned and begins

Which services, to what extent, at what times. We agree the plan with you. If something changes, we adjust it together.

What we can clarify in the first conversation

  • What support is needed – and how often?
  • Is there a doctor’s prescription, or does one need to be obtained?
  • Which documents do the health and long-term care insurance funds need, and who issues them?
  • Who pays which costs – and what needs to be applied for?
  • Is the address within our service area?
  • Can we take on care to this extent?

Whether we can take on care is clarified after this conversation.
