Intensive care transition

Safe transition to out-of-hospital intensive care.

For hospitals, social services and discharge management: how an enquiry for out-of-hospital intensive care at Rozali's Pflegedienst works.

When to contact us

A good transition does not begin on the day of discharge. You can get in touch early – even before the exact discharge date is set. The earlier we know the need, the better we can prepare the planned care together.

What we check first

  • Is the location within our service area (approximately 50 km around Wuppertal)?
  • Does the type of care fit our service offering?
  • Are there promising approval foundations with the payer?
  • Do we likely have staffing capacity at the desired time?

How an enquiry works

  1. First professional enquiry

You contact us with the care need in keywords: type of care, approximate scope, location, planned start. No patient data via the public form.

  2. Clarify care setting

We clarify the nursing requirements: ventilation situation, medical devices, required hours, special features.

  3. Check geography, fit and capacity

We check service area, needs and likely capacity and give you initial feedback.

  4. Clarify prescription, payer and contract

We clarify the required medical prescription, the payer's conditions and the contractual status.

  5. Clinical exchange via secure channel

Sensitive medical information is exchanged only after the first contact through an agreed secure channel – not via the public form.

  6. Planning and prepared care start

If care is possible, we coordinate discharge, staffing, medical devices and start timing with you and the family.

Which information may be relevant later

  • Medical prescription and diagnoses (via secure channel, not via the form)
  • Ventilation parameters and device settings
  • Nursing report or progress documentation
  • Payer details and planned transition information

Protecting sensitive data

Please do not send diagnoses, findings, lab results, discharge reports or other health data via the public form. The form is for first contact only. Once a trusted connection is established, we agree on a secure way to exchange medical documents.

An enquiry is not a commitment

Submitting the form does not mean acceptance of care. Whether a transition is possible depends on individual care requirements, geography, staffing capacity, payer conditions and clinical feasibility. We tell you clearly and early whether and to what extent care might be possible.

Submit a care enquiry

The professional partner form is sufficient for first contact. No patient data via the public form.
